Obsession v. Compulsion

Obsession: uncontrollable persistence of an idea or emotion in the mind; intrusive thoughts are recognized as senseless.

Compulsion: irrational force that makes someone do something, often unwillingly; repetitive behavior, ritual.

OCD v. OCP

OCD:
1. "Under the attack" of the obsessive & compulsive rituals.
2. Symptoms are painful; ego-dystonic
3. "Seized" by an impulse to carry the ritual; no pleasure, just relief.
4. NOT Impulse-Control Disorder (ICD)
[5. Axis I diagnosis - a mental illness]

OCP:
1. Perfectionism, emotional rigidity, devotion for work.
2. Part of oneself, ego-syntonic (i.e. harmony with ego).
3. Axis II diagnosis

OCD: Treatments
1. Behavioral therapy exposure & response prevention (i.e. systematic, gradual exposure & response prevention)

2. SSRIs
Dose needs to be higher than for other disorders.

3. Clomipramine (Anafranil)
Cyclic antidepressant.

Anorexia
Similar to obsessions & compulsions, but don't believe in the irrationality of the behaviors.

[Can use an SSRI for anorexia.]

No self doubts
No worries about self harm or harming others

Thoughts are unwanted, intrusive, inappropriate, uncontrollable.

1. Dirtiness, contamination, germs
2. Safety concerns
3. Incompleteness (i.e. lack of order or symmetry) [thinking they forgot something]

Extreme self doubt and worry regarding safety of oneself and others.
